It starts before the mouth. The appointment is the procedure, and it opens with the conversation, not the examination: a six block history across the four aetiological axes, nine validated screening instruments with their cut off points, and only then an examination guided by the hypotheses the conversation raised. You name a lesion by its location and investigate its mechanism separately. The mechanism does not go into the name.
Then you read the lesion as a signature, and the reason is timing. The contour alone names the mechanism. Texture and striae tell you whether it is still running, which decides whether restoring now is premature. And the mouth separates acid arriving from outside the body from acid from inside it, before the patient is asked anything. The doctrine it closes on: you do not control the lesion, you control the host.
Then the camera turns to the person. Risk does not add at the intersection, it multiplies: the patient carrying three moderate factors goes unnoticed because no single one is alarming. The work becomes order: which circle feeds the others, what changes first, what waits. You leave with a ceiling of one to two behaviour changes per appointment, and three tie breakers in fixed order: safety, continuity, reversibility.
