This 61-page English-language PDF workbook is for Canadian families sharing support for an adult. It keeps the person's priorities in the conversation while making transport, paperwork and other agreed tasks visible.

You will build a weekly plan with task owners, confirmed backups, a travel worksheet and a short factual handoff record. Chapters distinguish a request for help, a tentative booking and a confirmed arrangement. Journeys are planned as complete blocks of time, and uncovered work remains visible.

The workbook also addresses Canadian service contacts, the stages of an enquiry or referral, costs, human limits, privacy and changes during the week. Completed fictional entries and a full family case illustrate the process, followed by an action plan.
